Job Summary
The IUSM Department of Pediatrics Division of Pediatric Neurodevelopment Behavioral Center is seeking a Social Worker/Care Coordinator. The Social Worker/Care Coordinator works within a tertiary care coordination resource center to support primary care social work needs in the network; this includes making assessments, setting goals, and partnering with families in collaboration with the primary care coordinator. High quality coordination embodies the following care characteristics: patient and family-centered, proactive, planned and comprehensive, and promoting family empowerment.

Primary Responsibilities Include:

- Team Care Coordination: Work collaboratively with team members to distribute workload, utilizing and learning from each other’s skills. Assist in tasks within scope of practice when practice-based coordinators need help Support families’ needs to identify and understand the eligibility criteria for various health insurance/benefit/entitlement programs through federal, state and/or local public agencies. Review school reports and evaluations and provide support for patients and families in navigating the educational system to ensure patient is utilizing appropriate resources, services and supports. Assist patients and families with the identification of problems and obstacles to family living and community inclusion and develop strategies to mobilize community resources utilizing formal and informal supports.

- Family Consultation: Perform assessment; establish rapport with families to encourage goal setting; work with primary care coordinator to import information into the shared plan of care of care. Communicate effectively with involved families and providers. Perform outreach and referrals to new services, when indicated.

- Team Educator: Actively participate in cross training and team building with other members of the interdisciplinary team across the network of state practices, and provide education for other interdisciplinary team members as needed. Participate in professional development activities to keep current with social work and health care trends.

- Administrative Role: champions the social work scope of the Pediatric Care Coordination Resource Program to assist in program ongoing improvements, expansion and sustainability.

Required Qualifications
Master’s degree in social work (MSW) or a similar degree from a school approved by the Council on Social Work Educations and two years of experience dealing with infants and children with complex medical conditions.

Current Indiana licensure.

Counseling experience. Strong communication and team building skills. Computer and telephone skills. Skills in conflict management.

Preferred Qualifications
Prior experience partnering with professionals to support children with disabilities and their families.
