Job Summary

UHS is currently seeking a Primary Care Social Worker to assist primary care patients with management of medical needs, as well as administer depression and high-risk alcohol screening programs. The position will complement the current cohort of two mental health care managers and expand medical support to primary care patients and providers. Standards of practice for patient services offered by staff within this job family, have the potential to include care offered via telehealth.

Responsibilities*
Patient care coordination (90%)

Assist primary care patients with management of medical and mental health needs. In collaboration with patients and clinicians, facilitate access to transportation, home health care, and durable medical equipment. Conduct bio-psychosocial assessments and bridging appointments focused on new diagnoses, barriers to care, cultural factors, and treatment plan adherence. Connect patients to available resources that support optimal disease management/recovery. Interact with campus liaisons in Dean of Students; Environmental, Health & Safety; Athletics; Housing; and Dining in infectious disease outbreak setting. 50%

Administer depression and high-risk alcohol use screening program (PHQ9, AUDIT) in Medical and Women’s Health clinics. Ensure that all screening responses are entered in electronic medical record; respond to all high scores and indications of suicidality; identify patients at risk and facilitate access to care; help patients understand the results of their positive screenings and assist with referral, care coordination, and follow-up as needed. 40%

Administrative and liaison (10%)

Represent UHS in campus collaborative efforts (Alcohol and Other Drugs, Autism Spectrum Disorder).

Participate in benchmarking and quality improvement efforts surrounding primary care.

Supervise social work intern(s) in tandem with other social workers.
