Summary

The Department of Family & Community Medicine is seeking a qualified provider who will be responsible for medical oversight of a multi-site healthcare program for people experiencing homelessness which includes street outreach.  Directs the healthcare team to fulfill the mission and vision of HHH.  Serves as a member of the Leadership/Management Team. Accountable to the Chief Executive Officer.

Job Duties

Clinical:

  • Provide primary care as a medical provider to patients at Healthcare the Homeless-Houston as part of an integrated interdisciplinary care team


Administrative:

  • Responsible for completing/approving provider schedules to include
    • Clinic sessions
    • Vacation
    • Evening, weekend, holiday call schedule
  • Chair periodic provider meetings to
    • Discuss teamwork
    • Collaborate with the county hospital system (Harris Health) and mental health authority (Harris Center for Mental Health and IDD ), and other community providers
    • Discuss clinic policy
    • Implement new procedures
    • Review practice metrics
    • Update on formulary; immunization practices, community epidemiology, call, etc
  • Facilitate use of the Electronic Health Record
    • Insure standardization of forms/procedures throughout the clinics
  •  Supervise or delegate supervision of HHH medical providers (MDs, FNPs, and PAs)
  •  Oversee and develop physician volunteers for healthcare program
  •  Attend monthly meetings of the Board Health Quality Committee and give clinical update reports to the HHH Board of Directors
  •  Attend monthly Management Team and All-staff meetings
  •  Work with the HHH Director of Clinical Operations to direct clinical practices to accomplish the mission of HHH and develops the responsibilities of clinical staff to provide direct patient care in an integrated health home to fulfill that mission


Quality of Care:

  • Directs and coordinates all clinical aspects of medical care
  • Monitor effectiveness of current medical program and all clinical indicators, which may include:
  • Numbers and performance of staff
  • Provider productivity
  • Clinic capacity
  • Quality indicators as developed by HHH Staff and the Health Quality Committee
  • Cycle time
  • Responsible along with the Director of Clinical Operations for development, review, and implementation of clinical policies and procedures that conform to standards established by regulatory agencies


Customer Satisfaction:

  • Directly manage patient and staff complaints
  • Assist with patient/provider satisfaction surveys


Financial Managements:

  • Ensure efficient utilization of resources
  • Monitor physician and clinic performance


Education:

  • Incorporate resident and student training into practice
  • Oversee and help coordinate HOMES (student run clinic) - supervising MD of HOMES Clinic reports to HHH medical director
  • Participates in continuing education as required for professional license maintenance
  • Develop faculty skills in academic medicine


Community/Public Health:

  • Monitor local epidemiology for trends
  • Implement programs for control of priority public health areas such as influenza, cancer prevention, infectious disease control and women's & child health
Minimum Qualifications

Must have a medical degree (MD or DO)
Must be licensed or eligible to be licensed to practice medicine in the state of Texas
Must be Board Certified in Family Medicine or Internal Medicine
Must possess or be eligible to possess DEA license
Must possess BLS certification
2 - 3 years of direct clinical care experience, preferably working collaboratively in a multi-disciplinary clinical setting
Must be comfortable working with individuals and families experiencing homelessness and poverty
Must successfully complete the required credentialing and privileging process
