College/Department Information
Augusta University College of Nursing invites applications for our Lead Social Worker position. We are the state's flagship nursing program, enrolling more than 1000 students on two campuses and at numerous academic-practice partner sites. The College of Nursing, an integral component of the University System of Georgia is part of the state's only publicly funded comprehensive academic health science center including schools of medicine, dentistry, and allied health sciences. The College of Nursing is adjacent to Augusta University Medical Center, Children's Hospital of Georgia, Georgia Cancer Center, and the Veterans Administration Medical Center, providing ample opportunity for clinical experiences for students and faculty. Additionally, the college has a thriving Nurse-Managed Health Center, affording additional clinical practice and teaching opportunities focused on primary care, telehealth and behavioral health. The college offers the full degree spectrum including BSN, MSN, DNP, and PhD programs

Job Summary
Assists in setting the vision of Integrated Behavioral Health and supporting the Complete Care Program in collaboration with the Nurse-Managed Health Center leadership team. While also working to coordinate and deliver effective mental health and social work services by providing therapeutic interventions to patients and providing supervision and oversight to interns, social workers, and behavioral health consultants. Responsibilities include establishing policies, procedures, and protocols, developing, and implementing integrated behavioral health strategic plan, and establishing relationships with public and private referral sources and community partners. Responsible for collecting information from patients, families, and community agencies. Coordinate with the care team in the development, implementation, and evaluation of individual patient and family treatment plans. Gather information in regard to the specific community services available. To interview, assess and represent patients in order to meet their specific needs. To assess and assist with the acquisition of patient resources such as assistance with family services, child welfare, public health care and/or substance abuse management. Provide mental health services to patients and refer them to the PMHNP/Psychiatry as appropriate.

Responsibilities
Responsibilities to include but not limited to:


Administrative & Consultative-30%
Contributes to a high-performance team environment characterized by positive leadership and strong teamwork, including participation in interprofessional team meetings and project activities. Provides consultation to leadership regarding integrated behavioral health concepts and methods. Keeps abreast of current federal and state regulations, as well as professional standards, and makes recommendations on changes in policies and procedures to Clinical Director and Practice Manager. Serves as a key advisor to the NMHC Clinical Director, Practice Manager, and other key stakeholders in order to develop strategies and objectives to accomplish client and program goals. Develops training materials and provides consultative services to healthcare clinicians in recognizing and treating mental health conditions. Coordinates and implements staff trainings appropriate to their role in the integrated behavioral health model. Conducts training and provides learning opportunities to improve staff and care team competences. Participates in Advisory Team Meetings and other meetings as assigned. Develops policies and procedures related to the provision of integrated behavioral health services for the Nurse-Managed Health Center (NMHC).

Referral and Care - 70
Co-manages and collaborates with primary care providers and other interdisciplinary team members to assess patient needs and to develop and implement treatment plans. Interviews patients to gather health history data regarding their background, needs, and progress. Completes psychosocial assessments on all cases assigned. Utilizes evidence-based screening tools to collect data. Maintains expertise in common therapeutic modalities used in primary care settings which may include, but are not limited to, cognitive behavioral therapy, interpersonal therapy, motivational interviewing, problem solving therapy, and behavioral activation. Recognizes the signs, symptoms, and treatments of the most common mental health conditions. Communicates frequently with internal and external constituents regarding patient needs, response to treatment, concerns, and referrals. Provides appropriate evidence-based interventions to individuals, groups, and families. Collaborates in the development of appropriate goal-focused, person-centered, treatment plans containing measurable goals and objectives, obtaining patient and other stakeholder (guardians, family members, payers etc.) input as appropriate. Monitors, supports, and provides hands-on oversight in the delivery of behavioral health services and care management of patients. Maintains accurate and detailed records in the medical record, which includes entering, reviewing, and auditing records. Monitors, evaluates, and records client progress according to measurable goals described in treatment and care plan. Maintains communication as needed with internal/external constituents. Ensures objectives and priorities for patients are aligned with the care provided in the NMHC and in support of the Complete Care project goals and objectives specifically focused on the integration of behavioral and primary care health. Participates in quality and compliance reviews of assigned clinical records. Participates in utilization review of all assigned clinical records. Provides crisis intervention services within the scope of services provided at the NMHC. Refers individuals with complex or emergent care needs to appropriate care level for services needed. Works with departmental and NMHC staff to resolve safety issues for staff and patients.

Required Qualifications
Master's degree from an accredited college or university in Social Work conferred by a graduate school by the Council of Social Work Education licensed in the state of GA.

Knowledge, Skills, & Abilities
Knowledge of the mental health delivery system in Georgia, ability to perform crisis intervention and resolution, ability to utilize common modalities and therapeutic interventions. Ability to maintain confidentiality. Excellent interpersonal, written, and verbal communication skills. Proficient in Microsoft Office and other computer software/databases.

Shift/Salary/Benefits
*.80 FTE will work 32 hours per week.*
Shift: Mon-Wed,Fri 12PM-8PM. Thursday 12PM-8:30PM.
Pay Grade: 17
Salary Minimum: $44,967-$52,492 Annually.
Salary to be commensurate with qualifications of the selected candidate within the established range (generally minimum-midpoint) of the position.
