Summary

The Senior Practice Manager provides leadership and oversight in the accomplishment of strategic and operational objectives for Healthcare for the Homeless –Houston (HHH).  Directly responsible for clinical practice operations of a multi-site primary care practice for homeless individuals, including tactical plans for clinical services, personnel administration, financial management of clinic services, quality improvement/quality assurance, compliance, accreditation, clinical policies and procedures, eligibility, credentialing, and special project design and implementation.  Directly accountable to the CEO, with oversight from the Chief Medical Officer for clinical matters.

Job Purpose

Builds and maintains a work environment that promotes trust and engagement of competent team members and providers. Partners with the physician Medical Director, leadership team and various support departments to create a patient focused practice environment to drive progress and achieve goals in the areas of patient experience, quality, access, employee engagement, revenue cycle, financial performance, physician satisfaction, care coordination, patient and employee safety, EHR regulatory compliance, e-Health and other initiatives.

NOTE: The Faculty Group Practice will use the Clinic Management Complexity Grid (a point based system) to identify the level of management needed for a given clinic operation. 

Job Duties

Human Resources, Strategic Planning and Program Implementation

  • Oversees staff orientation and onboarding, training and continuing education to include in-services and community-based education.
  • Responsible for Change Management for clinical services.
  • Maintains relationships with external community partners to support strategic goals.
  • Participates in strategic planning and design for the agency programs.
  • Develops action plans and processes for programmatic changes or strategic shifts.
  • Implements and monitors changes to the clinical and operational programs of the agency.
  • Assigns tasks and provides guidance to personnel responsible for specific functions for programs.
  • Ensures timely employee credentialing to facilitate on-boarding and participation in insurance plans and government programs, that updates all licensure appropriately.
  • Acts as liaison maintaining communication between Baylor College of Medicine, Department of Family and Community Medicine and HHH. 

Financial Management and Grant-Related Reporting

  • Prepares and utilizes management reports to assess the efficiency, and productivity of clinical operations, and the status of programs and services; institutes any corrective action  plans. 
  • Maintains and monitors appropriate operating budgets and business strategies for each clinic site; conducts fiscal operations through cost containment.
  • Directs staff and clinical services to ensure effective use of resources, and monitors further need for additional staff, or new resources.
  • Leads in development and monitoring of billing practices to maximize revenue cycles.
  • Ensures compliance with regulatory guidance(s) affecting clinic services 
  • Prepares and interprets standard and ad hoc management reports; completes relevant sections of grant reports.
  • Tracks, monitors and reports clinical and patient measures and outcomes. 
  • Plans pre-audit activities to prepare for inspection visits from federal, state and local funding and regulatory agencies; implement auditing recommendations and verify readiness and compliance.
  • Serves as the point of contact for clinical vendor management.

Quality Improvement

  • Coordinates quality improvement activities with the QA/QI Coordinator.  
  • Assesses, develops, implements and monitors the quality improvement plans for clinical services.
  • Provides coaching on QA documentation to applicable clinical staff members.
  • Develops and monitors performance indicators and institute corrective action plans.
  • Reviews and updates Clinic Policy and Procedure Manual,  and creates new policies as indicated.
  • Assesses organizational systems, processes and workflows to improve efficiency, productivity and customer and staff satisfaction; identify opportunities for improvement and implement appropriate staffing and management changes. 
  • Responsible for education and training of staff on quality improvement programs and initiatives.
  • Maintains CMS and Patient Centered Medical Home (PCMH) designation. 
  • Investigates and responds promptly to patient, provider, practitioner and staff concerns/complaints.
  • Monitors patient satisfaction through routine evaluations, such as patient satisfaction surveys.

Electronic Health Record

  • Functions as an operational liaison with the EHR vendor. 
  • Collaborates with the vendor to ensure HHH functionality is maintained.
     

Minimum Qualifications

  • Bachelor's degree in Healthcare, Business, and Accounting or related field. Four years of related experience may substitute for degree requirement.
  • Three years of relevant experience in a related medical setting as a supervisor.

 

 

 

Preferred Qualifications

  • Master's degree in a related field.
  • Five years of progressive management experience in ambulatory setting, behavioral health and/or homeless/underserved populations.
