College/Department Information
The Medical College of Georgia (MCG) is one of the nation's largest medical schools by class size, with 304 students per class. The MCG educational experience is anchored by the main campus in Augusta, with regional clinical campuses for third- and fourth-year students across the state, and two four-year campuses located in Athens (in partnership with the University of Georgia) and in Savannah (in partnership with Georgia Southern University). MCG's expanding partnerships with physicians and hospitals across Georgia currently provide more than 350 sites where students can experience the full spectrum of medicine, from complex care hospitals to small-town solo practices. MCG and its teaching hospitals also provide postgraduate education to approximately 575 residents and fellows in 50 different Accreditation Council for Graduate Medical Education-approved programs. Our researchers and clinicians focus on what most impacts the health of Georgia's and America's children and adults, including cardiovascular biology and disease, cancer, neurosciences and behavioral sciences, public and preventive health, regenerative and reparative medicine, personalized medicine and genomics. Our physician faculty also share their expertise with physicians and patients at about 100 clinics and hospitals statewide.
Job Summary
This Nurse Navigator position will support the AU Center for Blood Disorders, an outpatient classical hematology clinic that serves adults with sickle cell disease, bleeding disorders, and clotting disorders. The Nurse Navigator is an experienced nurse who promotes quality, comprehensive, multi-disciplinary, evidence-based care. The navigator will perform ongoing assessments of patient needs, monitor and coordinate care, complete patient-level forms and letters, manage prescriptions and Prior Authorizations, promote patient adherence with clinic visits and treatments, manage patient messages, educate patients and families, and evaluate progress toward health care goals. The navigator will help coordinate specialty and outreach clinics and will serve as a liaison for our Center with community and healthcare partners. The nurse navigator will function as a back-up clinic nurse and will provide ongoing support for data management/surveillance and quality improvement efforts.
Note: Clinical functions supported by this position take place on campus, via telemedicine, and in clinics located across Georgia. Occasional travel may be required.
Responsibilities
The responsibilities include but are not limited to:
Care Coordination
Facilitate and promote quality, comprehensive, multi-disciplinary care for transitioning and adult patients with benign hematology conditions in local and outreach clinics.
Perform systematic, ongoing assessments of patient needs and develop individualized care plans. Identify barriers to care plans and coordinate interventions as needed.
Gather pertinent information about the patient's medical, psychosocial, and functional status. Communicate with the multidisciplinary team (neurology, orthopedics, physical therapy, dental, primary care, mental health, social work, etc.) as indicated to complete the needs assessment. Coordinate multi-disciplinary conferences as indicated.
Coordinate and facilitate appointments and patient contact with all disciplines and services.
Assist with ongoing assessments of medications, including calling in or electronically entering new prescriptions and renewals, obtaining prior authorization, monitoring prescription frequency, and communicating with providers, patients, and pharmacies regarding any prescription issues.
Assist with follow up on laboratory and other test results, communicate with providers, and facilitate any needed intervention.
Assist with management of patient phone calls and triaging patient needs.
Help complete paperwork or write letters in support of patients' insurance needs, work or school requirements, disability assessments, financial issues, or other.
Monitor patient status toward goals and evidence-based management.
Help schedule specialty and outreach clinics; help promote patient compliance with treatments and clinic visits; follow up on no-shows to clinic and referral visits; help monitor and improve census trends.
May require occasional travel to outreach clinics.
Education
Provide information / education to patients and family members in order to promote seamless, continuous, and comprehensive care in the patients transitioning from pediatric to adult care and in the existing and new adult patients in the sickle cell and bleeding disorder clinics. Initiate and document teaching and develop patient I family education programs or tools based on assessment of needs. Educate patients and families about research opportunities.
Serve as a resource for community educational events, such as health fairs, fund raisers, screenings, symposiums, lectures, and staff education.
Clinical Care
Serve as back up clinic nurse as needed for adult sickle cell and bleeding disorder clinics.
Maintain clinical databases - data entry, auditing, and reporting. Ensure clinical databases are current, complete, and accurate.
May require occasional travel to outreach clinics.
Other
Perform duties as assigned to facilitate clinic and research operations and to promote optimal patient care and patient satisfaction.
Assist with Quality Assurance/ Quality Improvement efforts.
Perform all other related duties as assigned.
Required Qualifications
Bachelor's degree in Nursing from an accredited college or university, active licensure as a Registered Nurse in the state of Georgia, and a minimum of two years of experience in an outpatient or hospital setting. Must possess a current American Heart Association Healthcare Provider BLS/BCLS certification.

Shift/Salary/Benefits
Shift: Days/M-F (work outside of normal business hours will likely be required of an employee in an exempt level position)
Pay Band: B14
Salary: $75,300/annually - $96.000/annually
Salary to be commensurate with qualifications of the selected candidate within the established range (generally minimum-midpoint) of the position
